Please download and complete our new appointment form to bring in 15 minutes prior to your appointment. This is needed due to our new computer medical records system, necessary to stay current in our office with the latest services.

Office Hours:

Monday and Friday: 8:00 am—5:00 pm
Tuesday—Thursday: 7:00 am—5:30 pm (Optical opens at 7:30 am)
Saturday: 8:00 am—12:00 pm (Labor Day to Memorial Day only)
Closed: 12:00–2:00 pm the first Wednesday of every month for a staff meeting.
